NEW PATIENT

As a patient registered at Castle Gate Medical Practice you will be able to book routine services at either of our Surgeries. As part of the Aneurin Bevan University Health Board, all our patients are entitled to direct referral to all NHS Wales services. The Practice gives its assurance that it will not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, disability, medical condition or appearance.

How to register with us:

You will need to complete a NHS family doctor services registration form (GMS1 form) for yourself and for each member of your family when you come to register at the practice (these are available to collect from reception in Monmouth and Raglan, unfortunately we are unable to provide electronic copies of this form). You will also need to complete a New Patient Registration/Health Questionnaire.

Please check our boundary map before you request to register as a new patient as we are unable to register you if you live outside this boundary.

We would ask all new patients to read our practice leaflet which sets out our responsibilities to you and your responsibilities to us. A new patient appointment will be offered to all new registrations.

Documents required when you visit the practice to register:

  • Registration form GMS1 (collect from the practice)
  • New Patient Registration/Health Questionnaire (collect from the practice or select above and print at home)
  • Official form of identification and proof of current address
  • Please also bring details of any regular medication you are taking

Patients with particular needs:

Both surgeries are accessible to patients using a wheelchair. We offer parking at our Raglan site. A wheelchair is available for use at both Surgeries. There is a “LOOP” system available for people with hearing difficulties. We have access to special telephone facilities for patients who have hearing difficulties and patients who do not speak English. If requested, information leaflets can be made available in large print or Braille. We can arrange for an interpreter if needed.

Temporary Residents:

You are able to see a doctor anywhere in the UK if you are away from home and in need of urgent medical help. You can do this by simply asking to see the nearest doctor as a temporary resident. If you have a relative/friend staying with you on a temporary basis, we will be happy to see them should they need to urgently see a doctor/nurse. Please telephone the surgery should you require further information. You will be asked to fill in a temporary resident form at the Surgery.
